The Navy ship commander who fired a M4 with a VCOG mounted backwards and KAC rail not fully seated has been relieved. As I said back then - based on the photo his Sailors likely intentionally set him up to be ridiculed by handing him a rifle set up like that. Probably a sign of what they thought of his leadership...
